It is also contraindicated in patients with cardiogenic shock, severe bradycardia, second or third-degree heart block, acute heart failure, or severe liver disease.
This combination can interact with antifungal - CYP3A4 inhibitors (e.g., itraconazole, ketoconazole), antibacterial agents - CYP3A4 inducers - (e.g., rifampicin), anti-convulsants (e.g., phenytoin), and CYP2D6 inhibitors - selective serotonin reuptake inhibitors (e.g, fluoxetine, paroxetine).
