Age and Dose Restrictions

• Pirfenidone is meant for use in adults over 18 years of age.

Contraindications

• Pirfenidone is contraindicated in patients with known hypersensitivity to Pirfenidone or any of its ingredients.

• It should not be used by patients with severe hepatic impairment or end-stage liver disease.

• Individuals with severe renal impairment (CrCl <30 mL/min) or end-stage renal disease should also avoid using this medication.

Other Medications to Avoid

• Pirfenidone may interact with CYP1A2 inhibitors (such as fluvoxamine and ciprofloxacin) and CYP1A2 inducers (like rifampicin and omeprazole).

Overdose and Missed Dose

• Overdosing on Pirfenidone may cause severe gastrointestinal disturbances like nausea, vomiting, and diarrhoea. If signs of overdose are observed, consult with a doctor immediately.

• If a dose of Pirfenidone is missed, it should be taken as soon as remembered. However, if it is close to the time for the next dose, skip the missed dose and continue as normal. Do not take a double dose to compensate for a missed one.

Management of Side Effects

Most side effects are temporary and generally harmless and resolve after discontinuing this medicine. However, if you experience any severe side effects or worsening of any of the symptoms, please consult your doctor.

• Ease nausea and vomiting by taking medication with food.

• Combat fatigue by taking rest periods throughout the day.

• Manage rash or photosensitivity by avoiding direct sunlight and using a broad-spectrum sunscreen.

Use in Driving and Operating Machinery

• Pirfenidone may cause side effects like dizziness or fatigue that could impair the ability to drive or operate machinery. It is advised not to engage in these activities until you know how this medicine affects you.

Use in Pregnancy and Lactation

• The use of Pirfenidone during pregnancy and breastfeeding should only be considered if the potential benefits outweigh the risks.

• Consult your doctor before using this medication during pregnancy or while breastfeeding.

Caution in Other Conditions

• Pirfenidone should be used with caution in individuals with hepatic impairment, as regular liver function tests are necessary.

• Even though no dosage adjustments are necessary, caution should also be exercised in patients with renal impairment.

Special Dietary and Lifestyle Recommendations

• Regular exercise and a balanced diet may help improve overall lung health.

• Quit smoking, as it can lead to further lung damage and decrease the effectiveness of Pirfenidone.

• Drink plenty of fluids to stay hydrated and ease some of the gastrointestinal side effects.

• Avoid excessive alcohol consumption as it may increase the risk of liver damage.
